He is survived by: his parents, Thomas M. Church (Andrea) and Cynthia Church (Pettit); his daughters, Olivia, Alicia and Julia Church; his siblings, Jackie, Ann Marie and Michael Church; and his pets, Kona and Reese. He is also survived by several aunts, uncles, nieces, nephews, cousins.
