He was predeceased by: his brother Charles Yusczyk. He is survived by: his wife Carol A. Yusczyk (Hoffman); his children, James Headley (Eileen), Susan Kuppel (Mark), Crystal Yusczyk (Thomas Metty) and Jason Yusczyk; his siblings, Phillip Yusczyk Jr. (Jennifer), Ronald Yusczyk and Cynthia Fleming-Johnston (Johnny); his grandchildren, Kayla, Caitlin, Hannah Metty, Julia, Keith Jr., Adam, Jessica Fort, Crystal Cintron (Justin), Kaylee Kuppel, Kenny, Kayle Pawlowski, Michael and Janelle Fagan; and his goddaughter Kim Cich. He is also survived by many great grandchildren, nieces and nephews.
