He was predeceased by: his wife Joan Butlak (Hagle). He is survived by: his children, Michele Eiss (Kenneth), Kathleen Pratt (Edward), James "Max", John (Lori), Alphonso "Al" (Lisa) and Michael (Kristine); and his sisters, Dorothy Cavanaugh (William) and Joan Miller (David). He is also survived by nieces and nephews.

If so desired, memorial contributions may be made to Hospice Buffalo in Alphonso's name.
