He was predeceased by: his parents, Stillman T. Cook and Margaret Cook (Allen); his son Richard Gaylord Cook (Rhonda Cook); his grandchildren, Tara Cook and Jimmie; his siblings, Gail Herman and Andrew W. Cook; and his stepbrother Sanford T. Cook.
He is survived by: his wife Janine Montante; his children, David (Julie Cook), Pamela Cook of Florida and Margaret (Jim Scott); his grandchildren, Elizabeth, Deanna, Kristen, Felicia, Ashley, Alexis and Carina; and his brothers, Gary Cook of Southern Tier and Tom Cook of Nunda. He is also survived by several nieces, nephews, cousins, and dear friends.
