He was predeceased by: his parents, Jake Cook and Fran Cook (Leaf) of Akwesasne; his brother Woodrow "Woody" Cook (Pam); and his brother-in-law Pete Rusello. He is survived by: his wife Donna Arbour; his sons, Jonathan Cook and Keenan David of Akwesasne; his mother-in-law Rita Arbour; his nieces and nephews, Jonathan, Noah, Katelyn Cook, Chrissy and PJ Rusello; his brother-in-law Paul Arbour; his aunts, Elda Cook and Leona Cook; and his godchildren, Paul Jackson and Tewathasarakie.

Memorial contributions may be made in his memory to Ronald Mc Donald House in Burlington, Vermont.
