He is survived by: his children, Donna Williams of Vail, AZ. and Roy Dean Formwalt of Santa Fe, NM.; his grandchildren, Tonnia Long, Trisha Cathey of Capitan, NM. and Jared Formwalt of Capitan, NM; his great grandchildren, Jada Long and Austin Cathey; his sister Mary Myrick; and his brother-in-law Robert Bartlett.
