He was predeceased by: his parents, Jack Perkowski and Myralon Perkowski (Sadler). He is survived by: his sister Dr. Lisa Perkowski (Jo Givens); and his nieces and nephews, Lana "Mayme" Fick-Perkowski, Ann Gonzales, David Gonzales and Frankie Gonzales.
