He was predeceased by: his children, Patrick Chavez and Kimberly Chavez; his parents, Manuel Chavez and Refugia Chavez (Cuca); and his brother Freddy Chavez.

He is survived by: his wife Isabel Chavez; his children, Jerry Chavez Jr., Shirley Cordova (Andy), Lorrie Ortiz (Rey), Eugene Chavez (Jenell), Juan Gallegos (Cindy), Cuca Chavez Salazar (John), Luis Chavez and Ralph Romero; and his brother Manuel Chavez. He is also survived by twenty-seven grandchildren; thirty seven great-grandchildren; one great-great-grandchild.
