He was predeceased by: his parents, Moses and Pauline. He is survived by: his wife Mary; his son Jamie; his grandchildren, Mia and Andrew; his son Jed (Kerry); and his granddaughters, Abigail and Morgan.

In lieu of flowers, the family kindly requests that donations be made to the Alzheimer's Association (alz.org) in Dave's name to help fight the disease he battled valiantly during the final years of his life.
