He was predeceased by: his mother Carrie Kaulback; and his siblings, Kathryn Grill, Dorothy Phillips and Walter Kaulback Jr.. He is survived by: his father Walter G Kaulback; his wife Nancy A Kaulback; his sons, Kurt Kaulback, MD (Maria) of Kyle Kaulback, MD and Kris Kaulback, MD; and his grandchildren, Eric (Iva), Kaitlyn, Kelsey and Colin. He is also survived by many nieces and nephews.
