He was predeceased by: his parents, John Mazurek and Irene Mazurek (Rospond); his son Kyle John Mazurek; and his siblings, Allan Mazurek and Annie Mazurek.

He is survived by: his wife Karen Vorhees Mazurek of Wellington, FL; his sons, Clayton James Mazurek (Ale) of Pembroke Pines, FL and Chad Matthew Mazurek of Fort Lauderdale, FL; his sister Donna Obal; and his granddaughter Kylie Mazurek.

Memorial contributions in honor of John may be made to St. Jude Children's Research Hospital.
