He is survived by: his wife Marie Forte of Somerset; his children, Jim Forte (Sunisa) of Las Vegas, Donna Peruta (Jim) of Wethersfield Connecticut, Tom Forte of Yuma and Dianne Suraci (Carl) of Hillsborough; his grandchildren, Danielle, Nick Peruta, Erica and Amy Suraci; and his siblings, Connie and Jerry (Linda). He is also survived by in-laws, nephews and niece.
Contributions in Jim's memory may be made in his name to The Salvation Army.
