He was predeceased by: his parents, Julius E. Shook and Wilma J. Shook; and his daughter Misty. He is survived by: his children, Amanda, Anthony, Matthew, Eric, Jeromy and Andrew; and his niece Sherry. He is also survived by his family, friends.
