He is survived by: his brother Dominick "Doc" Azzolini; his wife Pauline; his children, Gerald Azzolini (Maria), Patricia Tiernan, Donald Azzolini (Geryle) and Janet Smith; his grandchildren, Nicole Revfi (Brian), Erik Tiernan, Andrew Smith (Monica), Shelby Thomas (Michael), Gina Azzolini, DeAnna Azzolini (Matthew), Mallorie Azzolini and Caroline Poyner (John); his great grandchildren, Fiona, Desmond, Cooper, Isabella and Ivy; and his brother Mario (Irene).
