She was predeceased by: her husband Leonard Edward Polyak. She is survived by: her children, Leonard Polyak Jr (Michelle), Michelle Bayliss (Doug), Catherine Runfeldt (Alan), Edward Polyak (Reggie) and Richard Polyak (Julie); and her sister Barbara Keim. She is also survived by her 7 grandchildren and 3 great-grandchildren.
