He was predeceased by: his parents, Don Mayhew and Betty Mayhew (Uhland); his brother Don Jr.; his brother-in-law Woody Hemsley; and his grandparents, Francis Uhland, Frances Uhland (Simkins), Walter and Elsie Mayhew.
He is survived by: his wife Debbie Mayhew (Iles); his daughters, Denee Angelo (John) and Donica Sickler (Kregg); his grandchildren, Kolby and Kassaday Sickler; and his sister Debbie Hemsley (Mayhew). He is also survived by many nieces, nephews and cousins.
