He was predeceased by: his father Noberto Melendez. He is survived by: his son Mathew Melendez; his mother Nadia Cruz; and his siblings, Maritza Melendez (Leslie Reyes) and Denise Cruz (Antonio Rodriguez). He is also survived by many siblings, cousins, aunts, uncles, nieces and nephews.

In lieu of flowers the family is accepting memorial gifts.
