He was predeceased by: his parents, Madeline Kapushy and Gordon L. Cook, Sr.. He is survived by: his sister Mary Anne Basso; his brother-in-law Frank Passaretti; his wife Maria; his children, Jeffrey (Michele Doughty) and Christina Dacosta; his grandchildren, Madeline, Jack, Amelia and Alexandra Dacosta; his brother-in-law Joseph Passaretti (Cathy); his siblings, Judy Kaplan (Paul), Janice Tonks (Kevin), Patricia DeVuyst (Donald), Albert and Thomas Kapushy; and his sisters-in-law, Frances Scott (Thomas) and Julie Green (Ron). He is also survived by several nieces and nephews.
