She was loved and cherished by many people including: her husband Vincent R. Forma; her children, Vincent G. Forma (Kathleen) of Harrington Park, NJ and Robyn Armao (Anthony) of Park Ridge, NJ.; and her grandchildren, Charlotte, Annaliese, Gabriella, Vincent M., Arlene, Northvale and Montvale.
