He was predeceased by: his parents, Andrew and Martha; and his sister Margaret Connelly (Margie). He is survived by: his wife Kathleen Connelly; his sons, John "Jack" Connelly (Maggie), Brian Connelly (Kelly) and Patrick Connelly (Patricia Mahoney); his grandchildren, Brian Connelly Jr. (Kate), Kaitlin Connelly, Brianne Connelly, Patrick Connelly (Daria), Sean Connelly (Meghan), Elise Connelly and John Ryan Connelly; and his great grandchildren, Bryce, Augustus and Benjamin. He is also survived by his many brothers and sisters in-law, nieces, nephews, cousins, and dear friends.

Family prefers, memorial donations to be made in Jack's name to St. Ann's Church.
