He was predeceased by: his parents, Andrew Kozak and Anna Kozak; and his brother Edward Kozak. He is survived by: his wife Jean Kozak; his daughters, Janice Miller (Joe) and Joyce Young (Corey); and his grandchildren, Joey, Jordan, Jack, Jarrett Miller and Ryan Young.
