She was predeceased by: her parents, Oscar Schmidt and Mildred Schmidt; her siblings, Herman, Fred, Babe and Marie; her children, Randy Worthington and Janet Hazelgreen; her sons-in-law, John Hazelgreen and Nicholas Basso; and her grandsons, Nicholas Basso, Kris Worthington and John Hazelgreen.
She is survived by: her husband William Forsythe; her sister Eleanor Richmond; her children, Judy Basso, Fred Worthington, Ken Worthington and Ted Worthington (Celeste); and her grandchildren, Laura, Adam, Judy, Nichole, Mandie, Edward, Richard, Chrissa, Melissa, Vanessa, Theodore, Ken, Randy and Elizabeth. She is also survived by many great-grandchildren.
