He was predeceased by: his parents, Maria Perez and Santiago Ortiz; his sister Mercedes Ortiz; and his stepbrother Raphael Ortiz. He is survived by: his children, Melissa, Naomi, Luisa, Melinda Ortiz-Suber, Nerita, Alfonso Ortiz, Jr, Alfonzo Ortiz and Norman Hayes; and his siblings, Anna Santos, Ester, Georgina and Ofeila Ortiz. He is also survived by 12 grandchildren, 5 great grandchildren and a host of nieces and nephews.
