He is survived by: his mother Virginia Cook (Henderson); his wife Christina Cook; his daughter Brielle Cook; his stepdaughter Alycia Cooper; his brother Robert W. Cook (Giselle); his nephew Robert T. Cook; and his nieces, Melissa Zabriskie and Rebecca Campbell.
