He was predeceased by: his parents, Matthew Kozak and Susanna Kozak; his siblings, George, Steve, Tom, William, Dorothy, Irene and Evelyn; his son-in-law Andy Pisulak; and his son Edward Strohl.
He is survived by: his wife Celia (Canton); his children, Gerald R. Kozak of Metuchen, Kathy Pisulak of Lehighton, PA and Terry Strohl (Sandy) of Walnutport, PA; his grandchildren, Stephen Pisulak, Bethany Henry, Tanya Fliegel and Ashley Strohl; his great grandchildren, Hailey Pisulak, Natalie Pisulak, Harrison Guynn, Lillian Henry, Allison Henry and Logan Fliegel; his sisters, Susan Haines of Howard, PA, Millie Straw (Richard) of Idaho and Marcella Shaddick of Bradford, PA.; and his siblings-in-law, Richard Canton of Woodland Park, NJ, Lou Canton of Edison, NJ, Olga Miskovich of Metuchen, NJ, Lucy Mahalchick of Lambertville, NJ and Zina Canton of Santa Fe, NM. He is also survived by many nieces and nephews.
