He was predeceased by: his father John Costello. He is survived by: his mother Helen Costello; his close friend Janis Sullivan; his daughters, Paige Costello and Jill Snyder (Travis); his siblings, Helen "Tad" E. Costello (Jeffrey Patterson), John P. Costello and Bruce D. Costello (Kellie); his nieces and nephews, Caitlin, Colin, Megan, Jack and Ryan; and his grandson Dexter.
In lieu of flowers, memorial contributions may be made to American Cancer Society.
