She was predeceased by: her parents, Ann DeForge and Richard DeForge. She is survived by: her husband David McIntosh II; her son Ray Perruzza (Pam) of Lake Charles; her granddaughter Olivia Perruzza of Lake Charles, Louisiana.; her sisters, Maryann Grubesich (Jacob), Diane Amorino (Rich) and Carol Ann McCartney (Michael); and her sister-in-law Tracy McIntosh. She is also survived by her nieces, nephews, great nieces and nephews.
