He was predeceased by: his parents, Salvatore Facciola and Margaret Facciola. He is survived by: his wife Tatyana Dyachenko-Facciola; his children, Frank (Jennifer Facciola), Noelle (Jorge Collado), Alex (Yuri Dyachenko) and Salvatore Facciola; and his grandchildren, Michael, Ryan, Nicholas, Riela, Liam and Ellie.

In lieu of flowers, donations to the American Heart Association or the Emmanuel Cancer Foundation are greatly appreciated. (links below).
