He was predeceased by: his parents, Arthur Boonstra and Carol Anne Boonstra (Hulsebus); and his sister Laura Lynn. He is survived by: his wife Sue Boonstra; his children, Matthew Boonstra (Maggie Coakley), Amanda Faust (David) and Luke Boonstra (Bethany DeBlock); his grandson James Faust; and his siblings, Glenn (Jill), Jack (Linda), Mark (Michele) and Gregg (Jeni).

A memorial service was held on Friday, November 1st 2024 at 7:00 PM at the Unity Christian Reformed Church (339 N 11th St, Prospect Park, NJ 07508).

In lieu of flowers, donations may be made in David's memory to Cystic Fibrosis Foundation and Mahwah Fire Department Company 4.
