He was predeceased by: his parents, Joseph Maio and Carmela Di Maio; his wife Catherine Di Maio; his son David Di Maio; and his sister Phyllis Di Maio.

He is survived by: his sons, Joseph Di Maio and Anthony Di Maio (Janine); his grandchildren, David (Jerrilyn), Michael, Desi and Daniel Di Maio; and his children, Ryder and Hunter. He is also survived by 3 nieces.

In lieu of flowers, the family requests that donations be made to Our Lady of the Holy Angels Church, 473 Main Street, Little Fall, N.J. 07424 in Alfonso's memory.
