He was predeceased by: his parent Thomas E Provenzano II. He is survived by: his mother Carol Provenzano; his siblings, Randy Provenzano (Lynn), Brian Provenzano and Kimberly Provenzano (Craig); his niece Rachael Provenzano; and his nephew Vaun Provenzano. He is also survived by many aunts, uncles and cousins.
