He was predeceased by: his parents, Gladys Chadwick and Leo Chadwick; and his brothers, Donald and James. He is survived by: his wife Melba; his children, Raymond B. Chadwick (Zaydie) and Joy Lupo; his granddaughters, Melissa Lupo, Andreea Chadwick and Kailey Chadwick; his great grandchild Kylee Lupo; his siblings, Norman and Joan Third; and his children, God R. C. Church and NJ.
In lieu of flowers donations may be made in his memory to the Alzheimer's Association.
