He was predeceased by: his parents, Jeremiah "Grover" Blake and Elizabeth "Bessy" Blake; and his wife Carolann V. Blake. He is survived by: his children, Dr. Brian Blake (Annette) of Loudon, NH and Alyson Blake of Jackson Twp; his grandchildren, Brianna Allmann (Will), Casey Caprio, Abigail Blake (David Sheehy) and Hunter Blake; and his great grandchildren, William "Liam" Allmann and Blake Allmann.
