He was predeceased by: his wife Annie Lee Warren Lewis; his children, Alonzo "Lonnie" Lewis and Althea Lewis; his grandson Leonard Johnson; and his siblings, Maggie Jones, Mary Alice Fletcher, E. M. Mapp, Coris Mapp, Zonie Mae Mapp, Earnest Mapp, Louise Thomas, E. S. Mapp, Jr. and Lelar Shaw Mapp.
He is survived by: his son John L. Lewis (Hope) of Howell, NJ; his sister Willie Ruth Mapp of Decatur, GA; his brothers-in-law, Joseph Warren (Katie) and William Warren of Freehold; and his close friend Lula McBride. He is also survived by nieces, nephews, other family, and friends.
