He was predeceased by: his parents, Paul John and Sophie Hlavaty. He is survived by: his children, Lauren Ann Hlavaty (Annie) of Hoboken and Paul Dennis Hlavaty, Jr. (Joanne); his wife Katherine Hlavaty (Kathy) of Holmdel; and his granddaughters, Savannah Grace and Lilliana Sofie of Austin, Texas. He is also survived by nieces, cousins.
