He was predeceased by: his parents, John Boylan and Suzanne Schweikle-Boylan; and his siblings, Mike, Jack, Christine and Laura.

He is survived by: his children, Mariah S. Boylan of Leonardo, NJ., Matthew D. Boylan Jr. of Leonardo, NJ. and Miranda M. Boylan of Leonardo, NJ.; and his sister Theresa Flowers of South Bend. He is also survived by many other family members and friends.
