He was predeceased by: his parents, Anthony Kozierowski and Bernice Kozierowski; and his sister Joan Trovato. He is survived by: his wife Ximena; his children, Anthony Leonardo, Nicholas Joseph and Naomi Kaela Kozierowski; his brother Bill (Emily); and his close friend Antonio Campana. He is also survived by many other relatives and friends.

In lieu of flowers, donations may be made to the St. Jude Children's Research Hospital in care of the funeral home in loving memory of Anthony.
