He was predeceased by: his siblings, John and Joan. He is survived by: his wife Regina Ulaky; his children, Jim Ann (Sue), Mike Ann (Stephanie) and Mary Ann (Rich); his grandchildren, Michael (Casey), Steven, Jim, Kim Ulaky, Chris, Kelly and Alyssa Rebele; and his great grandchild Nora Ulaky.
