He was predeceased by: his parents, Franciszek Zak and Telka Zak (Kobylarz); his wife Waleria (Poltorak); and his siblings, Josef, Tomasz, Marcin and Maria.
He is survived by: his daughters, Anna Kotwica (Stanislaw) and Teresa Nieroda (Jacek); and his granddaughters, Agnieszka, Monica and Susanna.
