He was predeceased by: his parents, W. Peter Forman and Muriel Forman; and his pet Romeo. He is survived by: his wife Betty; his sons, W. Peter Forman (Jennie) of Succasunna and Brian Forman (Catherine) of Freehold; his grandchildren, Callie Forman of Peter John, Brian William Forman and Valentina Forman; his brothers, John Forman (Janice) of Farmingdale and Clayton Forman (Michele) of Myrtle Beach, SC; and his sister-in-law Patricia Tully (Joseph). He is also survived by many nieces, nephews and dear friends.
Visitation was held on Sunday, January 12th 2025 from 2:00 PM to 5:00 PM and on Monday, January 13th 2025 at 9:30 AM at the Clayton & McGirr Funeral Home (100 Elton-Adelphia Rd, Freehold Township, NJ 07728). A funeral Mass was held on Monday, January 13th 2025 at 10:30 AM at the St. Rose of Lima Catholic Church (16 Mc Lean St, Freehold, NJ 07728). A burial was held on Monday, January 13th 2025 at 11:45 AM at the Maplewood Cemetery (169 W Main St, Freehold, NJ 07728).
