He is survived by: his wife Carolyn L.; his children, Emily Polkowski (Benjamin Mainardi) of Vienna, VA and Lucas Polkowski of Gainesville, FL; and his siblings, Patricia Tocci and Joseph Polkowski. He is also survived by nieces, nephews, and cousins.
