He was predeceased by: his wife Beverly Enslin. He is survived by: his children, David, Diane D'Agostino Kaczka (Charles) and Daniel (Gina); his grandchildren, Megan, Sabrina, Shane and Alana; and his brother Thomas D'Agostino.

In lieu of flowers donations in his memory may be made to St. Jude's Children's Research Hospital, 501 St. Jude Place, Memphis, TN 38105.
