He was predeceased by: his parents, Grace Petrozzo Smith and Paul E. Smith, Sr.; and his in-laws, Helen and Pugar James. He is survived by: his son Jeffrey Patrick Smith; his wife Karalee Smith; his brother Paul Smith; his sister-in-law Dale; his nieces, Kelly Bretz, Tracy Bryce and Megan Smith; his siblings-in-law, Tom James and Virginia Jablonski; and his nieces, Gillian Galgan and Abby Hatch.

In lieu of flowers, memorial donations are being accepted in Patrick's memory to The Morris Educational Foundation by way of inmemof.org.
