He was predeceased by: his parent Andrej Kostialik. He is survived by: his wife Linda Lutcza; his children, John Kostialik (Tedra), Fred Kostialik, Kim Grosch and Andrew Kostialik (Jodi); his grandchildren, Janeen Pietraszkiewicz (Matt), Dustin, Thaddeus, Cassidy, Kendall, Jared, Andrew, Nicholas Kostialik and Douglas Grosch; and his great grandchildren, Kalei and Jakob. He is also survived by nieces, nephews, extended family, and friends.
