He was predeceased by: his parents, Alroy Perkins and Mary L. Perkins (McDaniels); and his brother John Wayne Perkins. He is survived by: his wife Susan Perkins (Chabina); his children, Michel J. Perkins and Brittany; his son-in-law Michael Werkheiser; his sister Carolyn Bunch; and his grandson Jack Michael Werkheiser.
