He is survived by: his wife Nancy; his son Kevin; his daughter-in-law Maria; his grandchildren, Kristen, Taylor and Matthew; and his brothers, Thomas and William. He is also survived by nieces, nephews.
In lieu of flowers, the family is requesting a donation be made in Jack's name to the Ronald McDonald House.
