He is survived by: his wife Ulana Kaliczynsky Prokopiak; his sons, Michael of Charlotte, NC and Thomas of Colonia; and his siblings, Patrick J. Prokopiak (Kelly Cawley) of Avenel, Joseph P. Prokopiak (Rebecca) of Toms River and Deborah L. Prokopiak-Gomez (Frank) of South Plainfield. He is also survived by many nieces and nephews.
