He was predeceased by: his son Andrew. He is survived by: his wife Maureen; his daughter Erin Fahy (Patrick); his grandchildren, Fiona and Mason; and his brothers, Paul (Dee) and Joseph Kolakowski (Colleen). He is also survived by his many nieces and nephews.
