She was predeceased by: her parents, Thomas Koslak and Frances Koslak; and her husband Americo Mazzariello (Rick). She is survived by: her siblings, Jean Vadimski, Pauline Lesinski, Chester Koslak, Helen Koslak, Mary Lipot, John Koslak and Frank Koslak; and her children, Spica Nap Usher, Kathleen O'Connor (Michael), Teresa Cuchiaro, Richard Mazzariello (Toni Marie) and Helena Mazzariello (Terry Nishimura). She is also survived by 10 grandchildren, and 7 great grandchildren.
