He was predeceased by: his wife Lorraine Kozak; and his sister Harriet Fritz. He is survived by: his children, John P. Kozak (Mary), Tom Kozak (Jessica) and Suzanne Keith (Ryan); his grandchildren, John, Matthew, Will, Jack, Kayla, Carly and Ryan; and his niece Janice Kulikowski.
